• Small patient-to-nurse ratio (10:1) ensures thorough, personal, quality care
• Nurse visits minimum 2x/week and may visit as often as needed (no cap on visits)
• Home health aide 2 times per week, may increase as needed
• Holistic approach in care: physical, spiritual, emotional, and family-centered
• All medications related to the terminal diagnosis and comfort are delivered to the patient’s home and paid for 100% by hospice
• DME related to the terminal diagnosis and comfort is provided
• Triage nurse and on-call nurse are available 24 hours a day to make visits PRN for patients/family distress
• Aggressive management of pain and symptoms
• Psychosocial and spiritual support: social worker and chaplain visit monthly and as needed; may assist with funeral planning
• Bereavement care for the family for thirteen months after the patient’s death
• Volunteers to provide, as needed, the vital companionship and support to patients and caregivers 
• Covered 100% by Medicare, Medicaid, and most insurances
• Hospice medical director makes home visits as needed and is available 24 hours/day for consultation
• Ongoing education for the patient/family on the dying process and symptom control
